Traditional surgical techniques relied on moving and removing structures in an attempt to recapture a youthful look. There were no good options for replacing volume which had been lost and many surgeons and patients did not care for the aggressive chemical and laser technologies available to them. We now have wonderful natural fillers like Restylane, Juvederm, Sculptra and Evolence to address volume loss reliably and with minimal downtime.

Light based technologies are now available which can impact color, texture and tone of skin. Fractional laser technologies allow minimal downtime and treatment of the neck and chest which were not treatable before with the full ablative lasers. Skin tightening is improving but still can not compare with surgical techniques. These tools now allow surgeons to address the recognized changes which accompany facial aging. It is the appropriate combination of these products and technology which create a true rejuvenation, making you look like you did when you were younger not just tighter or fuller.

After years of using these products I am convinced that prevention and maintenance are far better than doing nothing until major interventions are required to correct a problem. Being proactive with skin care, botox, fillers and now light based technologies will keep you looking better longer.

Botox has been a staple in aesthetic practices for 15 years and more indications are being discovered all the time. The toxin binds to motor nerves reversibly and prevents muscle contractions. When applied correctly and artistically, facial lines soften and relaxed, refreshed expressions return. Dysport was just approved by the FDA and is now available in our office. Dysport is a botulin A toxin which has been available in Europe and is now making its way to the States. Although the toxin is a Botulinum A toxin it is believed to have a faster onset and may have a slightly longer duration.

The number of fillers which have come to market over the last 3 years have enhanced our ability to contour the face with results which can last 12-18 months. These fillers consist of hyaluronic acid, collagen and in some cases synthetic materials which can be converted to collagen by the body. Restylane, Juvederm and Sculptra are the workhorse fillers in my practice. Each has a particular strength which makes them ideal for a particular correction.

Similarly, technologic advances with light based therapies have created devices which are yielding visible corrections with, in some cases, minimal downtime. The ability of fractional laser to improve skin tone and texture while treating the neck chest and hands has dramatically improved the usability of these devices. Although still not rivaling a facelift, the lasers are providing improvements which address issues that a surgical facelift could not. The issue of irregular skin color and poor texture can be improved reproducibly. I find these treatments particularly beneficial in 2 distinct groups. They are young women with early signs of facial aging who are not quite ready for a surgical procedure but want improvement and the older patient who has had a facelift to address skin laxity and facial shape but now needs improvement in the tones and texture of the remaining face and neck skin. I have been pleased with the improvements I have made with the DOT fractional laser and the Sciton BBL and Profractional laser.
 
By combining surgical and nonsurgical treatments the issues of facial aging can be completely addressed. The loss of elasticity of the skin, the loss of facial volume and improvement in the texture and pigment can all be addressed to yield a true rejuvenation.
